Entries: Entry Forms

The Organising Committee in association with the FISU International Supervision, Control and Arbitration Committee (CISCA) for the Championship will issue Accreditation Cards to those participants who have submitted all the required data to the Organising Committee.  To ensure a smooth and trouble free process, countries must ensure that all Individual and Nominative Entry Forms completed in full reach the Organising Committee within the prescribed deadline set by FISU, i.e. by 2nd August 2008. 

The following entry forms are relevant: 

N1       Nominative Entry Form for Competitors

N2       Nominative Entry Form for Officials

N3       Delegation Travel Schedule

I-1       Individual Competitor Entry Form

I-3       Individual Officials Entry Form

I-4       Individual Entry Form for FISU Family and Guests 

Entry Forms should be completed in the Roman alphabet in one of the working languages of FISU, i.e. English, French, Russian or Spanish. 

All individual entry forms must bear the name and signature of the National University Sports Federation (FNSU) representative as well as the seal or stamp of the responsible organisation of the country the player or official represents.  Entry forms without the necessary FNSU authorisation (signature and seal) will unfortunately not be accepted.  Only individual entry forms of participants from a non-member association must be countersigned by the concerning National Sports Federation (FSN) or Olympic Committee and stamped with a seal from the said organisation. 

If you wish to make changes to Entry Forms after submission, please notify the Organising Committee in writing of such changes immediately.  This will eliminate unnecessary delays and frustration in the issuing of your accreditation cards upon arrival at Sun City.

ARRIVAL AND REGISTRATION PROCEDURE 

Upon arrival at the Johannesburg O.R. Tambo International Airport, delegations and FISU Family members should report to the World University Golf Championship Information Desk situated in the International Arrivals Hall.  The Head of Delegation of each team will them be requested to identify each member of their delegation whereupon they will each receive a Temporary WUGC’2008 Pass that will grant them access to the official vehicles that will transport teams to Sun City.  Each competitor and official will then be requested to identify their luggage upon which they will receive a tag that must be attached to each luggage item.  ONLY persons in possession of a valid WUGC’2008 Pass and ONLY luggage items with the appropriate tag will be transported to Sun City. 

Upon arrival at the Sun City Cabanas, delegations should report to the World University Golf Championship desk from where they will follow the following registration procedure: 

· Confirmation by the Head of Delegation of the participants' list of his/her delegation;

· Registration of possible modifications;

· Taking of photos in the order of names appearing in the team list;

· Confirmation by the Head of Delegation of the departure date(s) of his/her delegation and the number of days accommodation required at Sun City;

· Payment by the Head of Delegation of all outstanding participation fees, including breakage deposit of US$300; (Alternative arrangements will be made for teams arriving after 22h00)

· The Head of Delegation fixes an appointment with the representative of the International Control Commission (CIC) of FISU in order to complete the accreditation procedure;

· Team members are taken to their rooms. 

The registration process for FISU Family members and guests will take place at the Sun City Hotel.
